health indicators such asmaternal and infant mortality have shown drastic improvements, alongwith a decreasing incidence of infectious diseases. there has been agrowing prevalence of noncommunicable diseases ncds, heavilyinfluenced by lifestyle-related factors such as poor diet, tobacco use,and excessive alcohol consumption as well as social isolation,contributing to growing incidence of mental health conditions, especiallyamong the older persons. the prc has undergone a rapid demographictransition and is a moderately aged society, on its way to become aseverely aged society by 2035. in 2020, it was estimated that thepopulation over 60 years accounted for 18.7 of the total population itis expected that by 2050, the population above 60 years will reach 35,in line with current trends of declining birth rates and reduced fertilityrates.the coronavirus disease covid-19 pandemic highlighted majordeficiencies in public health surveillance and management, andemphasized the importance of disease control as a regional public good.slow detection, response and reporting, and lack of coordinationbetween agencies, highlights the health system's challenge to detectand respond to pandemics and public health events, anddisproportionately affected older persons and the immunocompromised.lessons from the covid-19 pandemic indicate a vital need for moreresources at the community and primary health care phc levels todetect and manage outbreaks, and improve the capacity of publichealth and primary health care institutions to coordinate responses.located in the northwest prc, as of 2021, shaanxi has a total populationof 39.54 million. the population is aging quickly-the proportion of peopleaged 65 years and older increased from 8.71 to 13.99 from 2011 to2021. the aging population poses particular challenges in shaanxi dueto poor delivery of integrated health and elderly care, especially in ruralareas where populations are more widely spread. further, while dataintegration is critical for health and elderly care integration and qualityphc, many issues remain. telemedicine and treatment at phcs are alsounexplored, though it can improve access to health services in ruralareas, which has significant time saving impact for women. capacity ofphc services is also weak for ncd management, and disease detectionand response. failure to address these issues is a bottleneck forimproving health outcomes and disease control efforts, which has a far-reaching replicative impact for other provinces and has regional benefitssince health is a global public good.due to perceived poor quality, people commonly bypass phc facilities toobtain care in secondary and tertiary facilities which contributes toinefficiencies in the health system. this is further reinforced by ashortage of qualified health workers, leading to poorer prevention,diagnosis, erroneous referrals, and lack of care integration andgatekeeping of the phc system. furthermore, integration of phc withelderly care is weak, due to lack of standards, systems integration, andtraining, impacting the health of older persons. phc institutions shouldalso serve as the first point of contact with the population to providecomprehensive care to manage the health of the population of olderpersons, and play a critical role as sentinel sites in the public healthsystem to detect, control, and prevent infectious diseases, and otherpublic health threats such as those related to climate change. climatechange is also expected to negatively affect accessibility of healthservice provision by magnifying extreme weather impacts on healthservice infrastructure, and introducing vector-borne diseases where theywere previously less common and anticipated, impacting especially
